Overall Purpose:
The aim of the Transition form is to support students in the transition process in order to ensure relevant and appropriate support strategies are put in place where necessary. It is for all 14-19 young people in Trafford. It can be used at a number of points of transition:
Year 11 transition
When a young person either remains in their current place of learning or applies to other education/training providers
Pre-16 (14-16)
When young people undertake some of their learning at a different place of learning e.g. another school, a college, another organisation
Post 16
Between programmes e.g. E 2 E to college, college to work based learning
It is the intention that the contents of this document and the process through which it is managed is never to prejudice a young person's application or experience of their learning programme. This is not a selective tool but is a tool to support all young people and if used in the interview process it will be used in a supportive way.
The Transition form ideally needs to sit within the systems and activities around the transition process generally e.g. Progress File, Individual Learning Plans, Pathway Plans, Personal Education Plans, Section 140. Where appropriate the form will also feed into the Common Assessment Framework. There is the expectation that all young people will have a 14-19 Transition form as part of their 14-19 Entitlement.
For good practice at pre and post 16 it is the expectation that this form is completed with the young person whenever possible or they have sight of this before it is processed and a young person's consent is sought.
The purpose of this form needs to be clearly explained and confidentiality discussed.

The information that is provided is:
- Factual
- Constructive
- Evidence based
- Objective
Additional Notes for Clarification
UPN: Unique Pupil Number - a young person will have a UPN up to the age of 18.
Section 3 :
Current Provider : This covers any named organisation that is providing statutory and non-statutory provision that the young person is currently registered with. If they are not registered with any provider at this current time please tick NEET (Not in Education, Employment or Training) which is at the top of the form.
Section 6:
Trafford has adopted the NASSEA (Northern Association of Support Services for Equality and Achievement) EAL assessment system. This is based on QCA guidelines and has seven levels of English language acquisition.
First language - this is to support the assessment of need, for monitoring achievement, tracking and to ensure effective communication with home.

Section 8
Please record any qualifications/awards achieved or estimated grades (these may be, for example, a GCSE in Year 9) or SATs scores (for Year 9).
Sections 11 and 12:
This section relates to factors that may impact on learning. Health issues : could include pregnancy or health conditions having the potential for emergency medication e.g. diabetes and mental health issues.
Section 13:
Current support could include counselling, mentoring (please specify), 1-1 support because of their statement, attending (or recently attended) a support for learning programme (e.g. Personal Skill Building course), behaviour management programme, drugs programme.
Section 15:
It is important that young people have a clear understanding of the purpose, headings, process and useage of this form. Clearly there are some sensitive issues and these need to be recognised e.g. FSM entitlement - this is to ensure that this entitlement if relevant continues to be met e.g. during pre-16. Schools can share this information without the child's consent up to the age of 16. Once the child has left statutory education a young person's consent must be sought in writing for this document to be shared.

Process
The 14-19 Transition Form is to be completed by the end of January of Year 11 and at appropriate times for other transition points. The form will be available electronically via the schools intranet and the CYPS website under 14-19.
The home organisation needs to send the form to the relevant Learning Provider with a student's application (there is no expectation of this proforma needing to be completed if a young person wishes to remain at their current place of learning at the end of Y11).
Young people and parents need to be aware of the 14-19 Transition Form and where possible the form should be completed with the young person
The consent of a young person must be obtained if they have left statutory education
Forms need to be kept on file at school for any late applications at post 16. Until 31 st August the form belongs to the school and after that the proforma for any young person who has not completed a successful transition at 16 should be forwarded to Connexions
Currently at this stage the form is for Trafford Providers
The staff who have sight of this form will be determined on a need-to-know basis
